Items that were proposed, flagged, and then forgotten. No owner. No next action. No expiry. The Stale Item Index is not a backlog tracker. It is a thermometer. The carrier memory runs on attention as much as bandwidth. When items collect without resolution, that silence accumulates as friction. Every unclosed loop makes the next handoff heavier. Three items stand out: two show proposals from last cycle with no operator response. One catalog tag collision — duplicate entry, same hash, different names. None of these are emergencies.
